Job Summary:

We are seeking a highly skilled and experienced Clinical Pharmacist to join our healthcare team. As a Clinical Pharmacist, you will work closely with healthcare professionals to optimize patient care, provide medication expertise, and contribute to the development of evidence-based treatment plans.

Key Responsibilities:

1. Medication therapy management: Review and optimize medication regimens for patients, identifying potential issues and opportunities for improvement.

2. Collaboration with healthcare teams: Work closely with healthcare professionals to develop and implement treatment plans, providing medication expertise and guidance.

3. Patient counseling: Provide patient education and counseling on medication use, potential side effects, and disease management.

4. Medication safety: Participate in medication safety initiatives, identifying and mitigating potential medication-related risks.

5. Research and education: Contribute to research projects, educational programs, and quality improvement initiatives.

Requirements:

1. PharmD degree: Doctor of Pharmacy degree from an accredited program.

2. Residency training: Completion of a clinical pharmacy residency program .

3. Board certification: Board certification in pharmacotherapy or a related specialty preferred.

4. Licensure: Current licensure as a pharmacist in the state of practice.

5. Skills: Excellent clinical judgment, communication, and problem-solving skills.
